I have carefully read and fully understand the separate documents I have been provided entitled “Summary of Your Rights Under the Fair Credit Reporting Act,” “Disclosure Regarding Background Investigation Document,” “California Disclosure Document,” and “State and Local Notice Document,” as well as this Authorization Document. By agreeing to this Acknowledgment and Authorization Document, I consent to the release of consumer reports and/or investigative consumer reports (“Background Reports”) prepared by Evident ID, Inc., P.O. Box 19119, Atlanta, GA 31126, www.evidentid.com, 1-877-832-5298, Privacy Policy: https://www.evidentid.com/privacy-policy/ , or any other consumer reporting agency to Wonolo Inc. (COMPANY) and its designated representatives and agents for the purpose of determining and maintaining my eligibility to access and use the Wonolo platform as an independent contractor.

I understand that COMPANY may obtain Background Reports throughout my independent contractor relationship with them, if permissible under applicable law.
