Note that all appeals, including any documentation, must be submitted within 90 days of receiving the email notifying you of your account deactivation.

Please state in the Description field your reason for appeal and any information you wish the Wonolo Trust & Safety team to review in regards to your account deactivation.

This Appeal Form is available to covered app-based workers (as defined by SMC 8.40).
